REGISTRO VIRTUAL DE DOCUMENTOS COVID-19 PARA ACCEDER A LAS INSTALACIONES DE DP WORLD CALLAO

A partir del 01 de diciembre de 2020 entrará en vigencia el registro virtual de los documentos sobre COVID-19. Seguir las siguientes instrucciones:

1. Envíe los siguientes documentos según su tipo de ingreso a “[email protected]“. En asunto deberán poner NOMBRE APELLIDO / EMPRESA. Ej: CARLOS LOPEZ / INVERSIONES CRUZ S.A.C.

Tipo de ingreso y documentos a enviar:

• CONTRATISTA Y PROVEEDOR de DP World Callao
  • Prueba COVID - 19
  • Ficha Sintomatológica (15 días de vigencia y firmada por ingresante)

• AVITUALLAMIENTO / RECOJO DE RESIDUOS OLEOSOS O SÓLIDOS
  • Prueba COVID - 19
  • Ficha Sintomatológica (15 días de vigencia y firmada por ingresante)

• TÉCNICO DE REEFER EXTERNO
  • Prueba COVID - 19
  • Ficha Sintomatológica (15 días de vigencia y firmada por ingresante)

• AUTORIDADES (SENASA, APN, OSITRAN, ETC.)
  • Ficha Sintomatológica (15 días de vigencia y firmada por ingresante)

• AGENTE DE ADUANAS / AGENCIA DE ADUANAS
  • Ficha Sintomatológica (15 días de vigencia y firmada por ingresante)

• VISITA TÉCNICA DE PROVEEDOR/ VISITA EN AFORO / CONDUCTOR PARA AFORO
  • Ficha Sintomatológica (15 días de vigencia y firmada por ingresante)

• INDUCCIÓN DE SEGURIDAD / CAPACITACIÓN
  • Ficha Sintomatológica (15 días de vigencia y firmada por ingresante)

En caso tenga resultado positivo de prueba COVID – 19 deberá enviar ALTA MÉDICA EPIDEMIOLÓGICA adicional a los documentos solicitados líneas arriba.

2. Ingrese al formulario Online desde la siguiente dirección:

3. Terminado de llenar el formulario se le mostrará el siguiente mensaje, dar click en ENVIAR y estará completado su registro por 15 días calendario.
